Carbuncle Upper Arm in a diabetic patient – Dr Narotam Dewan, Dewan Hospital

Diabetes is a notorious disease, it lowers the immunity of the patient and makes the patient prone to infections. One such infection is the development of Carbuncle due to Staphylococci. It is usually present at the nape of the neck or on the back but it can develop rarely on other sites to in the body. This lady developed this on her upper arm. It is often accompanied by hyperglycemia and septicemia which have to be controlled too.
I can understand the concerns of the viewers about the outcome of scarring following a lavish incision and debridement of dead tissue. See the swelling carefully, redness and shine is covering a huge area around the central swelling. The color of the central swelling is distorted and blackish red. This indicates wide spread infection and necrosis of tissue and skin in the center. In diabetic patients the control of diabetes and infection go hand in hand, otherwise there is risk of septicemia, toxemia, ketoacidosis and diabetic coma. Rapid control of infection and tight control of diabetes is life saving and first concern of surgeon and doctor. Whereas healthy queries are welcome uncivilized bad language comments are uncalled for.
Had it not been for a phone bieng in an OR whilst my grandad was havng open heart surgery he would not have survived, once opened it was apparent that the procedure was too drastistic to to operate on at that hospital so they phoned through to guys hospital in london and were talked through the procedure so i say let them have phones, also other people need to get hold of them too.
NYC girl I
Dr Narotam-Dewan-Hospital Ludhiana after the surgery how long does it usually take for a carbuncle this size to heal? This one was really bad on the inside and I understand diabetics heal slower. My father and husband both are diabetic.
+pappppie Completely agree. You think like a smart Human unlike some messed up people on this site that feels the need to stomp on people they don’t know and probably would stomp on their own friends and family to make themselves feel better. HHMMMMcouph (GAZZAROVER)….

It’s funny how people watch a few youtube videos and suddenly they are a Doctor. If the actual Doctor buggered up I really doubt they would go posting the video on youtube.
I hope that the comments regarding this procedure do not intimidate or inhibit you from showing us more surgical procedures.
Thank you for showing us such a surgery is like. I am a retired nurse and I worked in trauma ER the only time I was in surgery was in school.
Thank you for such an interesting video. I didn’t know that removal of a Carbuncle differed from removal of a Cyst, but now I realize they are different.
This is a very interesting method, haven’t seen it before. Thanks for educating everyone!
Not mentioning any names on here but if someone can so easily attack another person in a comment with such vulgar and abusive language just because they may have a different point of view then in my opinion that individual is simply a miserable person in life. I believe that is called ‘bullying” but you could look it up on google too! Don’t bother wasting your words on me for voicing my opinion for what I read because they are my own person feelings.
I like to see the infection. I can’t see it when you use the suction. hose but good job!
Make a bigger hole!!
no way to get around a scare when you have these kind of infections…that tissue is dead and must be removed along with a little extra around the edge…very painful but one tissue is remove it feels almost good
It looks as though some skin needed to be removed as it may have been infectious or damaged therefore needing to be removed. scaring can be minimized with proper treatment once healed. I have an incision on my neck from cervical spine surgery and it’s pretty unseen unless pointed out! I’m sure the patient would rather have a scar then die from blood poisoning as such infections can get into blood stream if not treated.
I have had many surgeries. Most of them have been due to unstable or dislocating joints. I have had two levels fused at two different times plus a lobe of my thyroid removed. All scars were hidden in cresses but when you get older they are not that well hidden. I just think that all scars are part of living a long life.
Nice job. That was one nasty looking carbuncle 🙁
Were their loculations? Why was a general anaesthetic used?
Better clearer video.
wow,
GOOD job…..
Dr Dewan is the best person and surgeon I know. Thanks Dr Dewan for these wonderful videos. God bless you. Me and hundreds of my friends, in fact our whole College is you fan. You may remember that you operated upon me about 7 years back.
Why do a X cut? Why not a () cut? Just curious. Thanks ?
Zyona it’s a carbuncle. a cruciate incision is indicated for the drainage of carbuncles.
You got to get ALL the pus out, otherwise it will build up again…
?
thanks for this. WhY cut skin edges at 3:44 ?
+SanjeevGeorgeV I was debrided on the back of my neck as I became infected with the resistant staph aureus bacteria (MRSA) while getting a posterior cervical fusion, resulting in cervical osteomyelitis. The Doctor went easy on me, only light cutting. Ten days later I developed two abscesses next to my spinal cord. This time the doctor went in heavy and cut a lot away. My wound was tremendous. 8 weeks on a wound vac with 8 weeks worth of packing/unpacking. I can say that while the aggressive debridement left me with an ugly wound (2 inches long, one inch wide, down as far as possible) and has affected the use of my right arm, along with constant pain, my life was saved. My case was not complicated by diabetes. If possible i would post pictures of my wound, take my word for it, nothing pretty.
+SanjeevGeorgeV The edges are necrotic so debribed and this will keep the wound open longer to let pus drain out and heal from the bottom so a sinus formation is prevented.
Excellent procedure!
I stand by your expertise for this procedure. Very well done!
